چکیده

Upper urinary tract transitional cell carcinoma (UUTT) is a rare condition, and there are no internationally validated guidelines on how to treat these lesions. Invasive andmetastatic disease is more common than in bladder transitional cell carcinoma (TCC). Ureteroscopy with biopsy and computed tomography (CT) urography add diagnostic accuracy; however, in high-grade lesions, muscle invasion and lymph nodes may be missed in 10–40% of the cases. Open nephroureterectomy (NU) remains the gold standard against which other approaches should be compared. Laparoscopic NU is becoming widely accepted as a minimally invasive alternative, delivering similar oncologic outcomes at short or intermediate follow-up. For lowstage, low-grade lesions, for solitary lesions and in imperative indications, nephronsparing surgery (NSS) may be offered to the patient. The role of lymphadenectomy lies in correct staging, and its therapeutic benefit remains controversial. Ideally, large international, prospective trials should assess the best management of UUTT. # 2010 European Association of Urology. Published by Elsevier B.V. All rights reserved.

Clinical Significance of Lymph Node Dissection in Patients with Muscle-Invasive Upper Urinary Tract Transitional Cell Carcinoma Treated with Nephroureterectomy

We investigated the value of lymph node dissection in patients with cN0 muscle-invasive transitional cell carcinoma of the upper urinary tract (UUT-TCC). Medical records of 152 patients with cN0 muscle-invasive UUT-TCC, who underwent nephroureterectomy between 1986 and 2005, were reviewed. Sixty-three patients (41.4%) underwent lymph node dissection. The median number of lymph nodes harvested w...

Single-session laparoscopic cystectomy and nephroureterectomy

Patients with high grade and/or muscle invasive bladder cancer and with concomitant diseases of the upper urinary tract, e.g. urothelial tumors (transitional cell carcinoma - TCC) or afunctional hydronephrotic kidneys, may be candidates for simultaneous cystectomy and nephroureterectomy. Although the progress in laparoscopic techniques made these procedures feasible and safe, they are still tec...

Retroperitoneal lymph node dissection (RPLD) in conjunction with nephroureterectomy in the treatment of infiltrative transitional cell carcinoma (TCC) of the upper urinary tract: impact on survival.

OBJECTIVES To evaluate the prognostic impact of retroperitoneal lymph node dissection (RPLD) performed during nephroureterectomy on time to recurrence and survival in patients with infiltrative transitional cell carcinoma (TCC) of the upper urinary tract. METHODS The charts of 82 patients with T2-T4 TCC of the upper tract were retrospectively reviewed. The median patient age was 67.7 yr. Seve...

Laparoscopic Nephroureterectomy: Oncologic Outcomes

Purpose of Review: Laparoscopic radical nephroureterectomy (LNU) is being increasingly performed at several centers across the world. This review analyzes the published perioperative and oncologic outcomes of this procedure. Recent Findings: Laparoscopic radical nephroureterectomy (LNU) for upper tract TCC is performed pure laparoscopically (LNU) or hand-assisted (HALNU). The most debated issue...

Haemostatic partial nephrectomy using bipolar radiofrequency ablation

Objectives: To present our experience with laparoscopic radical cystoprostatectomy and bilateral nephroureterectomy for organ-confined, muscle-invasive transitional cell carcinoma (TCC) of the bladder in two patients with dialysis-dependent end-stage renal disease (ESRD). Patients and Methods: Two men aged 77 and 65 years with organ-confined, muscle-invasive TCC of the urinary bladder and pre-e...
